Review the PADI Diver Medical Questionnaire

Before confirming your diving course, we ask every student to review the official PADI Medical Questionnaire. It helps identify whether you may need medical clearance from a physician before taking part in dive training.

There’s no need to complete it now—simply review the questions and follow the instructions in the official form.

Before Booking

1 · Review the questionnaire

Read the official PADI Medical Questionnaire carefully before booking.

2 · If medical clearance is required

If the questionnaire instructs you to obtain medical clearance, download all three pages and have the Physician’s Evaluation completed before your course.

3 · Book your course

Once you have reviewed the questionnaire—and provided medical clearance if required—we’ll confirm your reservation and send your PADI eLearning and online registration.

4 · Complete your online forms

After booking, you’ll complete the official online PADI medical declaration and student registration before arrival.

Many participants do not require physician clearance. If the questionnaire indicates that you do, the required evaluation must be completed before diving.

Don’t worry: answering “Yes” does not automatically mean you cannot dive. Follow the instructions in the questionnaire to determine whether a physician’s evaluation is required.
